
Scheduling an Appointment and Medical Questionnaire
To schedule an appointment, please contact us by phone, email at meritum@meritum.lodz.pl, or via the form on our website.
Two days before the consultation date, we will call you to remind and confirm the upcoming appointment.
Before the first consultation, we kindly ask you to complete a medical questionnaire. It is important for us to ensure a safe treatment process, and we need information about your health condition, medications you are taking, and any allergies you may have. The questionnaire can be completed at the clinic or downloaded and filled out at your convenience at home. You can send the completed questionnaire to meritum@meritum.lodz.pl or hand it in at the reception before your visit. If you don’t complete the medical questionnaire at home, please arrive 15 minutes earlier than the scheduled appointment.
The necessary personal data for providing medical services include: first name, last name, PESEL (personal identification number), and address of residence. Please bring your identification document, and for parents, please prepare documents with the children’s PESEL numbers as well

Welcome to Meritum
Our dental clinics are located in the center of Łódź, at 101 Zachodnia Street and 44 Wschodnia Street. Patients appreciate them for their modernity and friendly atmosphere.
When you come for your appointment, we invite you to the reception, where our receptionists will be happy to answer all your questions and show you the waiting area.
In the waiting area, we provide patients with free WiFi, cold drinks, coffee, tea, and current magazines. If needed, complimentary single-use toothbrushes with toothpaste are available.

Welcome to the office
The doctor invites the patient into the office, where they first discuss the dental problems, past experiences, and treatment expectations.
Next, the doctor conducts a dental examination, diagnoses the patient’s dental condition, takes intraoral and smile photos necessary for treatment planning. If needed, they may refer for diagnostic tests (X-rays or computer tomography).
If you have current medical records, please bring them with you.

Individual Treatment Plan / Medical Consultation
After the examination, the doctor prepares a treatment plan considering the prices of the procedures. Depending on the needs, we conduct consultations within a multidisciplinary team. Consultations covering various dentistry fields (orthodontics, implantology, endodontics, prosthetics, treatment of temporomandibular joint disorders, surgery) allow us to select the best treatment approach for each patient individually.
We send the prepared treatment plan to the patient via email and discuss it directly at the next appointment.

Starting the Treatment, Appointment Schedule
After the visit, we invite you back to the reception to arrange a convenient date to start the treatment.
Before commencing the treatment, we perform a professional and comprehensive teeth cleaning to remove plaque and tartar buildup. This includes scaling (tartar removal), sandblasting (plaque cleaning), polishing, and fluoridation.
At Meritum, you can pay in cash, by debit/credit card, or in installments.
